What is Hospice Care?

When a patient is diagnosed with a terminal illness, their focus may shift from cure to quality of life. They may begin to explore their end-of-life options, and one of those options is hospice care. Hospice is a type of healthcare that focuses on the palliation (management) of a patient’s pain and symptoms as well as attending to their emotional and spiritual needs. This service prioritizes comfort and quality of life by reducing pain and suffering. The goal of halcyon hospice care is to enable the patient to live as fully and comfortably as possible for the remainder of their lives.

Nurse

Halcyon nurses provide expert medical care while honoring the person behind the diagnosis. They are highly trained professionals who balance clinical skill with compassion, ensuring comfort, dignity, and peace remain central throughout the hospice journey.

Our nurses monitor changes in condition, and support families by explaining what to expect and answering questions with patience and clarity. They work collaboratively with the full care team to ensure each patient receives thoughtful, individualized care that aligns with their goals and values.

Social Worker

We offer a licensed medical social worker who has specialized training in end-of-life care. As an essential member of the care team, our social worker can help facilitate discussions about difficult decisions, provide resources and referrals, and offer emotional support and recommendations to help improve the experience for our patients and their families.

Spiritual Counselor

Halcyon respects the religious preferences of our patients and their families. Based on your needs, you choose the level of care you desire to support the body, mind, and spirit. Our spiritual counselor is available to assist patients and families as they move through their personal goals regarding understanding end-of-life and their spiritual journey.

Hospice Aide

Hospice aides play a vital role in supporting daily comfort and preserving dignity. They assist with personal care needs such as bathing, grooming, and mobility, always respecting personal routines, preferences, and privacy.

Beyond hands-on care, hospice aides often become a familiar and comforting presence for patients and families. Their consistency, kindness, and attentiveness help create a sense of calm and normalcy during an otherwise challenging time.

Volunteer

Hospice care is provided by a team of trained professionals, but it also relies heavily on the work of volunteers. Hospice volunteers play a vital role in meeting the social needs of patients. They often visit patients and their families, providing companionship and emotional support. They may also help with practical tasks, such as running errands or helping with simple chores around the home. Hospice volunteers make a difference in the lives of both patients and their families, and they are an essential part of the hospice care team.

Massage Therapist

Halcyon believes in providing whole-person care to our hospice patients. We understand that physical comfort is an important part of managing stress and pain—and want to do everything to help our patients find comfort and peace. One way we provide support is by working with a massage therapist. Massage therapy is an effective way to reduce stress, relieve pain, and promote relaxation. Our massage therapist works closely with our patients and their families to develop a treatment plan that meets their individual needs. Pet / Music Therapy

At Halcyon Home, we believe healing and comfort are not limited to medical interventions alone. Pet and music therapy offer gentle, meaningful ways to support emotional well-being and connection.

Pet therapy provides companionship and comfort, while music therapy can evoke memories, ease anxiety, and create moments of peace. These services are thoughtfully integrated based on each patient’s interests and needs, supporting a holistic approach to end-of-life care.

Bereavement Counselor

Halcyon is committed to providing compassionate care for our patients and their families—before, during, and after losing a loved one. Grief isn’t linear and does not fit neatly into a predetermined framework; it fluctuates and is unique to each individual. It can be triggered by the memory of that summer vacation or a particular scent, and it can sneak up on us when we least expect it. We are often asked these questions regarding grief, and we understand their significance:

“What happens after hospice care?”
“What resources are provided to help with grief?”

No one should have to grieve alone. Our bereavement counselors are available for thirteen months following your loss, and our team remains available indefinitely should you need additional counseling or resources.

CAREOPOLIS

At its heart, Careopolis is about connection. It’s about bringing people together who care about each other and finding new ways to support one another. Your private, invitation-only website is easy to create and use, mobile-friendly, multilingual, and comes at no cost to you. A Careopolis is your central hub for communication and coordination between loved ones. By creating a profile on the site, each person can share updates, photos, and conversations. The site offers a calendar for tracking appointments and events and organizing help from family and friends. By using Careopolis, families can stay connected and coordinated, even when they are apart.
