Home health services
Get the care you need, when you need it most
The thought of receiving care in your own home is a reassuring one. Home healthcare can be a convenient and comfortable option for those who are unable to or prefer not to receive care in a hospital or nursing facility. For many people, home healthcare is the best option for meeting their needs. Though there are many physical benefits of home healthcare, there are emotional benefits, too. Receiving care in the familiar surroundings of home can help reduce anxiety and promote healing.
What is home healthcare?
Home health is skilled medical care in a client’s residence, as ordered by a physician. We understand that dealing with a medical condition can be difficult, and our goal is to make the process as easy as possible for our clients. If you do not already have an order for services, and you need them now or in the future following a planned surgery or for any other medical reason, please call Halcyon—we are here to help.

Social services
Social services provide a wide range of support for clients and their families. These services can help to coordinate care, provide education and resources, and connect clients with community resources. Home health social workers often work closely with other members of the healthcare team, such as doctors, nurses, and therapists. They also maintain close communication with clients’ families to ensure that everyone is aware of the client’s progress and needs. In some cases, home health social workers may also provide direct services, such as counseling or therapy. Regardless of the specific duties, our social workers play an essential role in supporting clients and their families during a time of need, such as:
- Advocate for individual clients by providing resources to address identified social and economic issues
- Serve as a liaison between clients and resources
- Help with problem-solving, coping, and development skills
- Provide grief counseling as part of the hospice process, including bereavement services following the loss of a loved one receiving Halcyon Hospice care
What’s the difference between home care and home health?
Home care assists with dressing, showering, dementia care, cooking, light housekeeping, medication reminders, driving, fall-risk prevention, companionship, and many other activities that keep a client happy, calm, safe—and as independent as possible.
Home healthcare focuses on providing medical treatment, while home care focuses on providing non-medical assistance with activities of daily living. For example, a home health nurse might provide wound care or give medication, while a home care nurse’s aide might help with bathing or dressing. Ultimately, the decision of which type of care to choose will depend on the specific needs of the individual and their family.
