What Is the Medicare GUIDE Program?
The GUIDE Model (Guiding an Improved Dementia Experience) is a Centers for Medicare & Medicaid Services (CMS) initiative that provides coordinated, comprehensive dementia care by integrating medical oversight, care coordination, caregiver education, and respite services to support individuals living with dementia and their unpaid caregivers.

Who May Qualify?
This program may be a good fit for individuals living with dementia who are receiving care at home and the family members supporting them.
To be eligible for the Medicare GUIDE Program, the person living with dementia must:
- Have Medicare Parts A and B (traditional Medicare)
- Have a dementia diagnosis confirmed by a clinician
- Not be enrolled in hospice, PACE, or Medicare Advantage
- Not reside long-term in a nursing facility
Participation is voluntary. There is no obligation to enroll.
